Submit U.S. Passport, must have at least 6 months remaining validity
2 Borneo Visa Application, fully completed and signed
2 Passport-type photographs required
Copy of most recent bank statement required
Letter from employer verifying employment or leave of absense
Copy of round trip airline tickets or itinerary
Letter explaining purpose of trip to IndonesiaSocial Visit Visa

If a multiple entries visa is required, an invitation from the host organization in Indonesia is required

A business letter of responsibility is required, to be typewritten on your company letterhead and addressed to: Consulate General of Indonesia, Visa Section, New York, N.Y.
The letter must state all of the following

The nature of the business performed
The name of the business reference to be visited
Guarantee sufficient financial support for the visit
Guarantee return transportation to the USA.

The business letter should be signed by a company officer/manager other than the applicant.
